Form No. 113 a (Where used)?

A cover that I purchased, posted from France to USA, intercepted in Bermuda (P.O. Label 904 applied). Inside the envelop was a Form No, 113 a (light blue) regarding one photo found missing. It is initialled "E.D.Q" and at the bottom is a code "B.P. 1165-42". I suspected the "42" is the year the form was printed. It seems the cover was posted in 1942. I cannot find a copy of this slip in Peter Burrows book on the West Indies. He does, however, comment on a Form No 113a on page 15 in the Barbados section saying that "I believe Form No 113a should be listed under Bermuda..." No image of the form is included in either Barbados or Bermuda sections. Could anyone verify if this is in fact a Bermuda slip?

Hello Michael
Page 28 of the Bermuda section illustrates the Form No. 113a.
The CONDEMNED label above it on the same page is Form No. 161
and there is a OPENED BY EXAMINER label Form No.149 (Bermuda page 13).
These suggest to me that your printed Form No. 113a is also a Bermuda item.
The printers' code BP could possibly stand for Bermuda Printers.
No clues from Peter Flynn's book.
All speculation I am afraid, but the best I can do.

Hello Michael- Sorry for the lateness of this response- This cover popped up as I was searching for covers in response to another member's question.

This Form 113a is from a registered letter mailed from Cayenne French Guiana May 10 1943 addressed to New York City,Transit marks from Miami (indistinct date) New York Foreign Branch 6/12/43 New York Tremont Station June 14, 1943. It was censored at French Guiana, Trinidad (PC 90 Form 167 censor #8043 and NY censor cellophane tape #9832. The form has hand written notation 8043 (censor number matching the label, 1/6/43 (June 1, 1943 and BS or BL initials.so it appears at least this example was inserted at Trinidad
